COVID Policies and procedures:
You agree to cancel your appointment if you have a fever, shortness of breath, coughing, or other symptoms associated with COVID-19 or if you have been exposed to another person who is showing signs of infection or has confirmed COVID-19 within the past two weeks. If you test positive for COVID-19 you will inform Carissa Harris @ Create Balance immediately.
o Shortness of breath or difficulty breathing
o Fever (above 100.4)
o Muscle pain
o Sore throat
o New loss of taste or smell
In order to provide in-person services, the following protocols must be followed:
1. Please text/call upon arrival, and wait in your car until called in by your practitioner for your appointment time. Temperature will be taken upon arrival. Temperatures of 100.4 or higher will be asked to reschedule.
2. Wear a mask inside the building at all times.
3. Remove your shoes upon arrival and place them away from anyone else’s belongings in the front room.
4. There is absolutely no waiting in the sunroom or other common areas.
5. Wash or Sanitize your hands as upon arrival and before departure.
The bathroom is cleaned after every use by practitioners. These cleanings are logged to support your health.
Practitioners are working with clients to ensure screening criteria are met (temperature checks, symptom check, exposure check) before admittance to the building is allowed.
Late Arrival Policy:
Time is valuable. I will honor your time, and I ask that you honor mine. If you find you cannot be on time, please call or text 303-475-5167 as soon as possible. I do my best to accommodate late arrivals. If you are more than 15 minutes late for your appointment, we may need to cut your session short or reschedule for another day.
I get it, Life happens! Please if you need to cancel an appointment I require 12-hours advance notice. Most efficient way to contact me is by phone call or text (303-475-5167) No shows and day of cancelations are subject to $55 charge. Fee waived for COVID related cancelations
If you are coming down with a cold, or sickness I prefer you keep the cooties to yourself and reschedule to a later date when you are well. I will not cancel on you unless myself or son are sick, family emergency or childcare falls through. After all family comes first. I will give you as much notice as possible!
Confirmation of Appointment:
You will receive a confirmation email when your appointment has been scheduled, this email will include the door code to get in. I like to send personal text reminders the day prior. At this time you may confirm or reschedule if need be at no charge to you. After confirmation has been made any no shows or cancelations are subject to a $55 charge.
Create Balance has the right to refuse service to anyone.
Payment: I accept Cash, Personal Checks, and major Credit Cards. A service fee of 3-5 dollars will be added if paying with Credit Cards. All returned checks are subject to a $30 fee, and future checks will not be accepted. Unfortunately, I do NOT accept insurance.
